Thank you for agreeing to participate in a case study for PPL PRS Ltd.   We would like to use the answers and information that you provide (including any quotes and any photos or videos taken as part of the case study) (the “Materials”) in certain marketing and PR materials.

To help you understand how your information will be used, please take note of the following:

• We wish to retain and use any or all of the Materials for the purposes specified below, and on the basis explained in this Consent Form.
• In giving consent, you acknowledge that it is being given on a continuing, worldwide and royalty-free basis and that it permits (but does not oblige) PPL PRS to use the Materials. You also confirm that you are authorised to give such consent, that the information in the Materials is correct to the best of your knowledge and belief, and that PPL PRS does not require any other consents from any other parties in order to use the Materials for the specified purposes.
• If in future you wish to request any changes or updates to the case study (e.g. if the underlying information becomes out of date), PPL PRS will act reasonably in relation to any such request but is not obliged to incur any cost in relation to the same and may instead choose to discontinue use of the Materials.
